Case Note Safety Nets

Two safety nets to protect clinical case notes against accidental wipes (e.g. select-all and a stray keystroke replacing a long note with a few characters).
What's New
• In-editor confirm dialog if a manual save would replace most of a substantial note's content
• Post-save Undo toast for 30 seconds whenever a save significantly shrinks the body
• Server-side recovery snapshots: prior body is captured before high-risk updates, capped at the 3 most recent per note. If anything still slips through, recovery is now a single DB query instead of a full Atlas snapshot restore
Good to Know
Triggers are tuned to only fire on near-certain accidents, so normal editing is unaffected.
