We have rebuilt our survey builder from the ground up to make client-facing forms far more flexible and easier to create.
What's New
• Drag and drop: build multi-page surveys visually, reorder and duplicate questions and pages, and preview exactly what the client sees on desktop and mobile.
• 17 question types, including text, number, address, choice, dropdown, scale, slider, date of birth, file upload and signature.
• Smart logic: conditional questions and skip logic adapt the form to each client's answers, with required fields and validation built in.
• Connected data and theming: link answers to Client Data Fields, add custom completion pages and match your practice's look.
• Send and review: assign surveys during onboarding or triage, let clients complete them in their portal, and review responses in the client profile.
Good to Know
Publish and draft controls keep live forms protected from accidental edits. Available to all accounts.
