Onboard / Triage SMS Content Refresh and Customisation Support

The new default SMS template is now

This release also now introduce a simple customisability for any practice to reword this triage SMS content.
Things to note: Customisability is pretty low except for rewording, i.e. there won't be any complex linguistic support such as plural vs singular and logics.
