Updates email sending + Email Settings

Along with this change is the introduction of customer-specific mailboxes, where we've assigned a unique email address to each of our customers, which will be used as the sender address for all customer communications (appointment notifications, report/letter sharing, etc.) from now on.
The customer's mailbox can be seen in the Control Panel -> Email Settings tab, where they are also able to change the sender name and reply to address of the emails. We've defaulted these to the practice's name and email address. Please note that customer mailboxes (From Email) cannot be changed by the customer, and they will have to contact us in order to change it.
