Service Disruption Log
Welcome to our Service Disruption Log, a historical record of past incidents that have impacted our services. This log is designed to provide transparency and insight into the challenges we’ve encountered and how we’ve addressed them. Here, you’ll find detailed information on each disruption, including the date, duration, impact, root cause, and the corrective actions taken to prevent future occurrences. We are committed to maintaining the highest level of service reliability, and this log reflects our ongoing efforts to improve and learn from each incident.
We classify issues as follows:
Disruption - a problem or issue with a critical part of our capability set. This may include isolated feature level issues or other problems that impact your experience using Tacklit.
Outage - a complete failure of either CareOS and/or our Engage applications, or part there of that is so critical it is deemed an outage even if other parts still accessible.
